Smiths Falls Nurse Practitioner-Led Clinic

The SFNPLC is recognized as a model organization providing comprehensive primary health care to thousands of patients, many of whom were previously unable to find a primary care physician. Their team-based approach is led by the Board, with Client Centered Governance® principles at the core of their leadership practice. Their programs and services continue to expand, as they collaborate with partners to achieve their Vision - Better Health Together

Meals On Wheels (Sudbury) retained the Client Centered Governance ® Team to support the agency in governance and leadership development over several years. Together, the team bolstered governance effectiveness through CCG training for the Board, ED and EA, earning the agency CCG Certification. By-law renewal, Board Policy development, and support on specific areas of ensuring quality governance and decision-making was completed as well. 

The agency has thrived and is meeting the needs of more clients, and has been a community hero through the pandemic. Here are some highlights:

  •  2021 : Certificate of Recognition from the Ontario Provincial Parliament.
  • Client base growth to over 600, regularly surpassing over 35 000 meals annually
  • New programs: Grocery Program & agency-Production of Hot Meal Replacements
  • Ontario Trillium Foundation:  $150 000 in funding
  • COVID-19 pandemic success in ensuring most vulnerable adults safe and fed. Included: delivered 45,999 meals to 541 clients / 35, 685 were hot meals and 10 314 meals were hot meal replacements. This is an increase of 37% from 2019 prior to the pandemic. 764 deliveries for groceries to clients.
